Zytux Injection Dosage

Chronic Lymphocytic Leukaemia: 375mg/m² IV on Day 1 of Cycle 1, then 500mg/m² on Day 1 of Cycles 2–6 (in combination with fludarabine and cyclophosphamide).

Rheumatoid Arthritis: Two infusions of 1,000mg separated by 2 weeks, repeated every 6 months based on clinical assessment.

GPA/MPA: 375mg/m² IV once weekly for 4 consecutive weeks (in combination with glucocorticoids).

Premedication with paracetamol, an antihistamine, and sometimes a corticosteroid is typically administered before each Zytux infusion to reduce the risk of infusion-related reactions. The first infusion is given at a slow rate, with the rate increased if no reactions occur.

Important Precautions

Hepatitis B Screening: All patients must be tested for the hepatitis B virus (HBV) before starting Zytux. Active hepatitis B infection may be a contraindication. Carriers require antiviral prophylaxis and close monitoring throughout and for several months after treatment.

Live Vaccines: Vaccination with live or live-attenuated vaccines must be avoided during Rituximab treatment and until B-cell recovery is confirmed after treatment completion.

Infections: Zytux causes B-cell depletion and suppresses immunity. Patients should be monitored for signs of serious infection throughout treatment. Any active infection should be treated before initiating Zytux.

Cardiovascular History: Patients with pre-existing heart conditions require cardiac monitoring during infusions due to the risk of arrhythmia and other cardiac events.

Pregnancy and Breastfeeding: Rituximab can cross the placenta and may cause foetal B-cell depletion. Effective contraception is required during treatment and for 12 months after the last dose. Breastfeeding should be discontinued during treatment and for at least 6 months after the final infusion.

PML Risk: Any new or worsening neurological symptoms (cognitive changes, weakness, vision changes) during or after treatment must be investigated urgently for PML.
